Every state that requires a contractor license also requires some form of insurance proof as part of the application or renewal process. The minimums vary by state, by license classification, and sometimes by project type. Below are the key requirements for the most active contractor licensing states. The Certificate Holder Issue — More Important Than You’d Think
Submitting a certificate with the wrong certificate holder name is one of the most common reasons license applications are delayed. Each board has a specific name format they require on the certificate. California’s CSLB wants: State of California Contractors State License Board, PO Box 26000, Sacramento, CA 95826. Florida’s CILB uses a slightly different format. Arizona’s ROC has its own.

The board then triggers a license suspension notice. Some states automatically suspend; others give you a 30-day cure window. Either way, you’re in jeopardy.
Reinstatement after a lapse usually requires new proof of insurance, a reinstatement application, and fees. In some states, you may also face a hearing before the board, particularly if the lapse resulted in consumer harm or uncompleted work.
